Request a sick note for work



Important: Self-certification

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website

If you are unable to use the PATCHS online consultation tool to request a repeat sicknote then please contact the surgery via phone.

Request a sick note by phone

Once you have been absent from work for more than one week you can obtain a sick note from your GP. Please contact the Surgery on 01704 531114 to request a routine telephone appointment, a consultation will need to take place if you have not been seen by a Doctor before for the current problem and it is not a continuing condition.

If the problem persists and you require a further sick note, you must contact the Surgery, a member of the reception team will ask a series of questions which will then be reviewed by a clinician prior to the sick note being authorised or should a telephone consultation be required, the Practice will contact you to arrange a further appointment.

Sick notes can be collected at the Practice or alternatively emailed or sent via SMS text message.
